About the role

The Outreach & Care Coordinator Manager oversees the day-to-day activities of CCs in the market, provides coaching and support, and manages performance. They also foster PCP partnerships, build community partnerships, and maintain knowledge of relevant programs and resources.

Responsibilities

  • Provide direct supervision of CCs' day-to-day activities.
  • Participate in executing daily CC tasks alongside team members.
  • Lead and support CCs with coaching, mentoring, and support.
  • Work with fellow local lead roles and care team members to ensure workflow execution.
  • Review data dashboards to manage CC performance.
  • Hire, train, deploy, and support local CCs.
  • Foster PCP and community partnerships.
  • Maintain knowledge of relevant programs and resources.
  • Contribute to the local leadership team.

Requirements

  • Deep commitment to improving healthcare for underserved populations.
  • Experience supervising teams of care coordinators, patient navigators, customer service, outreach specialists, or similar roles.
  • Cultural and personal humility.
  • A strong understanding of Medicaid/MCOs and barriers to healthcare access and quality among diverse, underserved populations.
  • A keen eye for detail and proven track record of effective monitoring and optimizing metrics.
  • Collaboration and communication skills in a multidisciplinary team setting.
  • Experience with healthcare data and/or Electronic Health Records.
  • A person who thrives on building and supervising a team of care providers, building professionalism, and fostering a positive culture.
  • Ability to travel the local area across Chicago to help establish new teams and support existing teams.

Qualifications

  • Bachelor’s degree in health administration, health management, public health, business administration, or related field.
  • Long time resident of the Greater Chicago area and knowledgeable of MCO and community resources.
  • Preferred: Multilingual skills.
