About the role

Urban Pathways is a catalyst for transformative change in the lives of unhoused adults, providing essential support and fostering an environment where individuals can redefine their narratives and become their best selves.

Responsibilities

  • Engage with unhoused individuals in New York City, building rapport and linking them to essential services and support within the community.
  • Conduct homeless outreach and comprehensive assessments of clients’ needs, strengths, and goals through continuous engagement.
  • Provide intensive case management, ACT Team referrals, and ongoing support to individuals throughout their transition.
  • Coordinate and monitor services with internal referrals and third-party providers, including medical, mental health, housing, employment, and social support.
  • Develop individualized service plans in collaboration with clients and relevant stakeholders.
  • Escorts clients to appointments, including medical, housing, and entitlement appointments.
  • Maintains accurate and timely documentation of client activities, interactions, progress, and outcomes in electronic database systems.
  • Communicates professionally both verbally and in writing, attends staff meetings, training, and supervisory sessions, and responds to inquiries in a timely manner.
  • Works flexibly, including weekends, holidays, and evenings.

Qualifications, Education And Experience

  • High school diploma or GED required. Associate’s degree preferred.
  • Experience working with unhoused individuals or populations with complex needs (mental health, substance use, etc.)
  • Strong knowledge of community resources, housing programs, and social services in NYC.

Skills & Competencies

  • Computer skills and knowledge of Microsoft Office (Word, Outlook, etc.).
  • Excellent communication, interpersonal, and crisis intervention skills.
  • Ability to work independently and as part of a collaborative team.
  • Commitment to maintaining confidentiality and upholding ethical standards in delivery service.
  • Bi/Multi-lingual a plus.
