Responsibilities
- Functions as the MIHP Care Coordinator (40%):
- Performs home and office visits, completing psychological, sociological, medical, and nutritional assessments according to protocol.
- Provides counseling as deemed necessary based on assessments.
- Participates in team care plan development and monitoring through case conferences with all disciplines working with the client, including other agencies involved.
- Makes referrals within and outside of GLBHC programs for needed services.
- Provides or arranges follow-up with other team members.
- Completes and submits all required documentation for client records in compliance with MIHP standards.
- Complies with billing and record-keeping requirements of the program (30%).
- Participates in quality improvement activities (10%).
- Performs other duties as assigned by supervisor (10%).
Requirements
- Associate’s Degree in Nursing required; Bachelor’s or Master’s degree preferred.
- Current Michigan RN license under Part 172 of Michigan Public Act 368 of 1968, as amended.
- CPR certification required.
- Experience:
- MSN or BSN with at least one year of community health, pediatric, and/or maternal/infant nursing services, or
- ADN with at least two years of community health, pediatric, and/or maternal/infant nursing services.
- Previous fieldwork in the service area preferred.
- Must be a licensed driver with proof of personal liability and property damage insurance.
Skills
- Able to communicate effectively with a diverse population.
- Ability to coordinate patient services and collaborate with other service organization personnel.
- Organized and effective with limited direct supervision.
- Ability to provide services in an empathetic, nonjudgmental, and objective manner.
- Comfortable visiting clients in their homes, including deteriorated or high-risk neighborhoods.
- Flexible in scheduling and accepting referrals from other members of the care team.
Physical Effort
- Must be able to sit, stand, and/or walk for an entire workday.
- Must be able to lift, carry, push, pull, and/or twist while holding up to 25 lbs. frequently.
Schedule
40 hours per week; flexible and varied.
Travel
- Extensive travel required within the service area using a company or personal vehicle (mileage reimbursed at the corporation’s existing rate).
- Minimal out-of-service area travel may be required for conferences/training (reimbursement consistent with GLBHC policy).
