MDS Coordinator RN
Salary range: $36 - $44/hour

We are looking to invest in an MDS Coordinator RN to support a top-rated skilled nursing and rehabilitation facility in Illinois, proudly holding the prestigious Gold Seal of Approval® Certification for Post-Acute Care and Nursing Care from The Joint Commission, the nation's most respected healthcare accrediting authority.
As an MDS Coordinator, you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
Requirements
- Unencumbered State of Illinois R.N. License
- Graduate of an accredited nursing program
- C.P.R. Certification
- Prior experience as a MDS Coordinator
- One (1) year of experience as an R.N. in a skilled nursing setting
- One (1) year of experience as a C.N.A.
