Outpatient Therapist PHP / IOP Adolescent Services - Part-time

About the role
The Counselor/Social Worker provides quality case management and psychotherapeutic services to adolescent patients (ages 13-17), as well as adult psychiatric, adult chemical dependency, and child patient populations (ages 4-11), along with their families, spouses, or significant others. This role serves as a member of an interdisciplinary team, supporting the organization’s treatment program and philosophy to ensure the delivery of high-quality care.
Responsibilities
- Conduct psychosocial assessments by interviewing patients and their family members to document history and identify preliminary treatment issues.
- Provide group, family, and process-oriented therapies, as well as educational and didactic groups for patients and family members using various professional treatment modalities.
- Document daily progress notes summarizing patient progress toward treatment goals, including contact with family, significant others, and community agencies.
- Develop and coordinate individualized discharge plans by utilizing treatment team and chart information to determine aftercare needs.
- Identify and assess family or community resources, such as group homes or mental health practitioners, to meet patients’ aftercare needs.
Requirements
- Master’s Degree in social work, counseling, or a related field.
- Experience in an inpatient or outpatient mental health setting preferred.
- Active LPC or LCSW license required.
Schedule
Part-time, Monday - Friday, 2:00 PM - 7:30 PM.
