Outpatient Therapist - Adult IOP Part-time

Brentwood Hospital is looking for a dynamic Outpatient Counselor to join our team of professionals dedicated to providing help and hope to individuals and families, improving their mental health. The role involves creating a safe, nurturing environment and offering options for continued improvement.

About the role

The LPC/LCSW provides quality case management and psychotherapeutic services to a diverse patient population, including adults (psychiatric and chemical dependency), adolescents (13-17), and children (4-11), as well as their families, spouses, or significant others. This position serves as a member of an interdisciplinary team, supporting the organization’s treatment program and philosophy to ensure quality care.

Responsibilities

• Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
• Provides group and family process-oriented therapies, as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities.
• Documents daily progress notes summarizing the patient’s progress toward treatment goals; records contact with family, significant others, and community agencies in progress notes.
• Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine aftercare needs.
• Identifies and assesses family or community resources such as group homes or mental health practitioners to meet patients’ aftercare needs.
• Willing to travel to satellite clinics to provide support when needed.

Requirements

• Master’s Degree in social work, counseling, or a related field.
• Experience in an inpatient or outpatient mental health setting preferred.
• LPC or LCSW license required.

Tuesday, Wednesday, Thursday 5:00 PM - 9:00 PM
