Responsibilities
- Provides clinical care within the RN scope of practice, documents assessments and reports moderate to distressing symptoms to Palliative Physicians/APPs and floor nurse
- Follow up on hospice GIP patients as needed
- Able to participate in advance care planning discussions with patients and families and completes documentation (advance directives)
- Communicates plans of care to referring providers and unit clinical teams and ensures recommendations are implemented
- Provides education to patients and their families about palliative care
- Functions as a resource and support to patient care units, and patient care staff regarding palliative care including medication delivery and end of life care
- Identifies learning needs of patient care staff and communicates these to Palliative leadership
- Provides education to hospital staff on Palliative Care as requested
- Professional communication skills with providers, hospital staff, patients/families, internal and community resources
Requirements
- Bachelor's Degree
- 3-4 years’ experience in working in academic or large community hospital in a related clinical area (oncology, geriatrics, ICU, neuro)
- Knowledge of palliative care
- 3-4 years palliative care/hospice experience
- CHPN specialty certification

Benefits
Structured orientation of approximately four weeks, including clinic onboarding and oncology clinic shadowing.
Pay
Hourly rate based on experience and qualifications.
Schedule
Monday - Friday, No Weekends and No Holidays Hours: 8:00am - 4:30pm
