About the Role
Coordinate patient care requests received by phone for palliative care and serve as a resource to staff. Review all incoming faxes to the office, answer pharmacy calls, and handle all medication refill requests. Coordinate physician orders (e.g., home health, hospice, hospital) and act as a liaison between the physician/provider and other facilities, as well as between the physician/provider and patients.

- Coordinate patient care and patient documents
- Perform nursing duties
- Serve as a resource for all staff
- Perform other duties as assigned
Requirements
- RN or LPN with a Louisiana State Board of Nursing license
- Phone triage experience highly preferred
- Oncology, infusion, and/or hospice experience highly preferred
- Knowledge of pain management and oncology nursing preferred
