Registered Nurse - Palliative Care

About the role

Mercy Home Health, a member of Trinity Health at Home, is seeking a dedicated Full-time Registered Nurse for our Palliative Care program. This position is based out of Bucks County PA.

Responsibilities

• Deliver skilled nursing care in the comfort of patients’ homes
• Create individualized care plans
• Administer medications
• Educate patients and families
• Support recovery and independence

Requirements

• Bachelor’s degree in Nursing (BSN)
• Current PA RN License
• CPR Certification
• Hospice/Palliative Care Certification
• Home care experience
• Strong clinical knowledge base
• Experience in hospice preferred

Qualifications

• Strong interpersonal, organizational, problem-solving, critical thinking, and priority-setting skills
• Ability to work independently and as part of a team
• Proficient in computer skills
