Responsibilities
- Conducts AWVs under physician supervision, including vital signs, vision/hearing screenings, falls, cognitive impairment, depression, and functional assessments.
- Updates patient medical and family histories, performs medication reconciliation, and updates living wills or Medical Power of Attorney documents.
- Creates and implements prevention plans for patients with complex and co-morbid conditions.
- Schedules AWVs, runs quality reports, collaborates with clinic/office staff, and physicians to improve quality benchmarks.
- Collaborates in outreach and management of patients with multiple health issues.
- Provides education and counseling to patients regarding their medical conditions.
- Prepares educational materials for patient use and contacts medical insurance companies for authorization.
- Ensures all necessary testing and scheduling is completed before necessary procedures or surgeries.
- Works with health service coordinators to ensure all necessary pre-authorizations, testing, and scheduling are completed.
- Provides specialized nursing assistance in quality, education, and research efforts.
Requirements
Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
Obtain certification in Basic Life Support within 30 days of hire date.
Qualifications
Bachelor’s degree in Nursing (BSN) preferred.
Skills
- Knowledge of and appropriate application of the nursing process.
- Working knowledge of procedures and techniques involved in administering routine and special treatments to patients.
- Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others.
