The model focuses on better primary care rather than service volume, with physicians spending more time with patients and providing comprehensive care in state‑of‑the‑art neighborhood clinics.

Responsibilities

• Provide outpatient primary care only.
• Spend more time with each patient to deliver comprehensive, high‑quality care.
• Lead and collaborate with a supportive care team to address patients’ medical and social needs.
• Focus on building relationships with a small patient panel.

Requirements

• M.D. or D.O. degree.
• Board certified or board‑eligible in Internal Medicine or Family Medicine (required).
• Fellowship training in Geriatrics welcome but not required.
• Active, non‑probationary state medical license.
• Highly collaborative with a patient‑centric orientation and experience leading care teams.
• Confident in delivering high‑quality primary care, including availability to patients and addressing medical and social needs.
• Driven to care for patients with comorbid conditions, the elderly, and/or medically underserved populations.
• Passionate about work and energized by being part of a fast‑growing organization.
