Responsibilities

• Mentor and lead a care team
• Provide high-quality primary care, including addressing patients' medical and social needs
• Collaborate with a supportive care team to ensure comprehensive patient care
• Manage a small panel size to focus on building strong patient relationships
• Be available to patients and address their needs
• Deliver care to those with comorbid conditions, the elderly, and/or medically underserved populations
• Drive the organization's mission of providing exceptional care to seniors

Requirements

• M.D. or D.O. graduates
• Board certified/eligible in Internal Medicine or Family Medicine (required)
• Fellowship training in Geriatrics welcome, but not required
• An active, non-probationary state medical license

Qualifications

• Confident in delivering high quality primary care

Skills

• Excellent communication skills
• Strong leadership abilities
• Ability to manage a busy schedule while maintaining quality care
• Experience with cloud-based electronic health records
