Outpatient Primary Care Advanced Practice Provider (NP/PA)

Responsibilities

• See patients for preventive care, chronic disease management, and acute visits
• Collaborate closely with a team of physicians, APPs, and clinical support staff
• Manage your own patient panel with opportunities for continuity of care
• Work a 40 hour/week schedule with minimal call responsibility

Qualifications

• Licensed and certified as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Illinois
• Board-certified in Family Medicine, Adult-Gerontology, or related field
• Strong clinical assessment, communication, and documentation skills
• Commitment to providing high-quality, compassionate care

Pay

Competitive compensation and comprehensive benefits package including a reasonable estimate of $115,000.00 to $130,000.00 on an annualized basis.
