Advanced Practice Provider (NP/PA) - Primary Care - Outpatient

About the role

The Medical College of Wisconsin’s Department of Medicine, Division of General Internal Medicine, is seeking a Nurse Practitioner or Physician Assistant to join the Primary Care Outpatient Services at Froedtert Hospital Specialty Clinics in Milwaukee, Wisconsin. The APP will function as a member of a multidisciplinary team and contribute to excellence in patient care, safety, research, quality improvement, teaching, and program administration.

Responsibilities

• Collaborate with an interdisciplinary team in diagnosing and treating conditions requiring general internal medicine intervention in an outpatient setting.
• Manage primary care needs, acute concerns, and co-morbid conditions such as diabetes, hypertension, heart failure, asthma, and anemia.
• Write accurate and timely progress notes, procedure notes, and clinical documentation.
• Facilitate communication with patients, families, and staff to promote continuity of care.
• Use evidence-based knowledge to guide clinical decision-making for wellness, women’s health, and acute and chronic conditions.
• Conduct post-hospital and emergency department follow-up visits.

Qualifications

• Advanced Practice Nurse: Master’s Degree or higher in Nursing from a nationally accredited organization.
• Physician Assistant: Bachelor’s Degree or higher in Physician Assistant Studies from an ARC-PA-accredited organization.
• Minimum Requirements: Advanced Practice Nurse/ Advanced Practice Nurse Practitioner Wisconsin RN License, Wisconsin Advanced Practice Nurse Prescriber License, American Heart Association Advanced Cardiac Life Support (ACLS), American Heart Association Basic Life Support (BLS), DEA registration, and certification by NCCPA.
