About the role
The position is located at Sports Medicine and Rehab, 60 Memorial Medical Parkway, Palm Coast, FL 32164. The schedule is Monday - Friday, 8am - 5pm.
Responsibilities
- Determines physical therapy diagnosis and establishes a plan of care, making recommendations for referral if treatment is needed outside the scope of physical therapist practice.
- Documents and charges accurately, completely, and timely using measurable and objective terminology following standards of practice, insurance guidelines and department/hospital policy and procedures.
- Participates in marketing to physicians and physician practices to build referral relationships.
- Performs evaluations and re-evaluations using recommended techniques and standards.
- Supports consumer marketing and public awareness through participation in community events, educational lectures/speaking, and/or other extension of clinical expertise to the community.
- Adjusts and adapts to changes in patient census by altering working hours to assist in other centers and programs as approved and observed by the manager or director.
- P Practices principles of professional development by maintaining clinical competencies, using best and/or evidence-based practices, and serving as a clinical instructor.
- Maintains productivity standards as defined by the department, through effective time management and clinical efficiency and adjusts work hours to meet the needs of the patients as required.
- Designs and implements treatment plan of care utilizing practice standards, recommended techniques, modalities, insurance guidelines, and patient needs.
- Assesses patient ability to safely and effectively perform functional mobility skills and evaluates muscle, nerve, joint, and integumentary systems.
- Maintains productivity standards as defined by the department, through effective time management and clinical efficiency and adjusts work hours to meet the needs of the patients as required.
- Monitors patient response and progress to treatment, modifying the treatment plan as appropriate.
- Performs other duties as assigned.
Requirements
- Education: Doctorate in Physical Therapy (Preferred)
- Experience: Experience treating Neurological Disorders (Preferred)
Qualifications
- Basic Life Support - CPR Cert (BLS) [Required]
- Physical Therapist (PT) [Required]
- OR Physical Therapist - Temporary (PT-TEMP) [Required]

- Proficiency in clinical assessment and treatment techniques
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team

Pay
The anticipated minimum pay rate for this position is $36/hour. The pay range is $65,622.66 - $122,055.21.
Schedule
Full time, Day shift.
