Outpatient Pharmacist (Meds to Beds) - Part-Time w/benefits

About the role

The Outpatient Pharmacist provides professional pharmacy services by reviewing, interpreting, monitoring, and dispensing medications as prescribed by providers and according to professional standards and practice. Pharmacists will perform comprehensive medication management and patient-focused pharmacy services in a high‑touch, ambulatory environment. This role supports pharmacy operations across meds‑to‑beds, home delivery, internal medicine clinics, and specialty care settings. The pharmacist works closely with providers, clinical pharmacists, care teams, and patients to optimize therapy, improve adherence, and ensure safe, effective medication use.

Responsibilities

• Review, interpret, monitor, and dispense medications as prescribed by providers
• Perform comprehensive medication management and patient-focused pharmacy services
• Support pharmacy operations across meds‑to‑beds, home delivery, internal medicine clinics, and specialty care settings
• Work closely with providers, clinical pharmacists, care teams, and patients
• Optimize therapy and improve adherence
• Ensure safe, effective medication use
