Patient Care Pharmacist - Meds to Bed/Retail - Part Time - Days

Responsibilities
- Patient Care Services:
- Review medication orders for appropriateness and adjust regimens based on protocols and interdisciplinary team consultations.
- Monitor medication therapy for potential or actual drug interactions and outcomes.
- Collaborate with healthcare providers during interdisciplinary patient care rounds and oversee pharmacy resident and student participation.
- Perform and/or review medication histories, document medication reconciliation in medical records, and facilitate the discharge process.
- Ensure compliance with the inpatient drug formulary and complete medication profiles, considering patient-specific factors.
- Coordinate and distribute medications, communicate pending issues, and ensure compliance with departmental policies.
- Provide direct supervision of pharmacy technicians, interns, and students, and provide drug information/education to health care providers and patients.
- Quality Improvement:
- Report adverse drug events and ensure compliance with organizational policies and procedures.
- Maintain competency in pharmaceutical knowledge and skills, and participate in departmental and hospital committees.
- Monitor compliance with state and federal laws and regulations related to the practice of pharmacy.
- Training, Education, and Community Service:
- Provide direct supervision of pharmacy technicians, interns, and students and assist with their education and training.
- Offer in-services and lectures to health care team members and conduct drug information and disease prevention programs for consumer and patient groups.
- Precept and evaluate pharmacy residents and PharmD students on residency/clerkship rotations.
Requirements
- Education: Bachelor’s degree in Pharmacy or PharmD degree required.
- Experience: At least 18 years old with no current drug or alcohol addictions/convictions, two years of health care pharmacy experience preferred.
- Licenses/Certifications: Current active Pharmacy Licensure (PH) for work state required; American Heart Association or American Red Cross Basic Life Support (BLS) preferred; American Heart Association Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) preferred; completion of an ASHP accredited residency training program and/or certification preferred.

Job Description
The pharmacist provides medication therapy management services in collaboration with other providers to ensure drug therapy is safe, appropriate, cost-effective, and efficient. They maintain competency in unique aspects of drug therapy for neonates, pediatrics, adults, and geriatrics, and patient dispositions (inpatient, ambulatory, and outpatient).
The pharmacist also coordinates distribution systems, prepares and distributes medications, and documents activities related to medication reconciliation. They ensure compliance with the department's and organization's policies, rules, and regulations, including state board of pharmacy guidelines and other applicable regulatory bodies.
The pharmacist serves as a liaison for inpatient patient information for outlying patient needs and participates in departmental and hospital committees as assigned. They provide direct supervision of pharmacy technicians, interns, and students, and provide drug information/education to health care providers and patients upon request.
