Outpatient Pharmacist - Full Time

About the role

The Pharmacist ensures accurate and timely distribution of medications and other pharmaceutical supplies according to established procedures and standards, in support of high-quality patient care. Responsibilities include interpreting, evaluating, and verifying medication orders while assessing for drug-drug, drug-allergy, and drug-disease state interactions. The role involves providing drug information to clinicians and patients, maintaining accurate records on patient profiles and controlled substances, and managing pharmaceutical inventories, emergency drug supplies, and medication security.

Outpatient Pharmacists additionally dispense medications to fill written and e-prescriptions, provide patient counseling, and monitor medication therapy. Both roles direct and verify the work of Pharmacy Technicians, support organizational goals through quality customer service and performance improvement, and pursue professional growth opportunities.

Responsibilities

• Interpret, evaluate, and verify medication orders for accuracy and safety.
• Assess drug-drug, drug-allergy, and drug-disease state interactions.
• Provide drug information to clinicians and patients.
• Maintain accurate and complete records on patient profiles and controlled substances as required by law.
• Manage pharmaceutical inventories, emergency drug supplies, and medication security on nursing units.
• Direct and verify the work of Pharmacy Technicians.
• Maintain competency and participate in clinical programs such as Anticoagulation Management, Pharmacokinetic Monitoring, Adverse Drug Event Monitoring, patient care rounding, pharmacotherapy optimization, and medication safety.
• Support organizational goals through quality customer service, performance improvement, and teamwork.
• Actively pursue professional growth and development.
• For Outpatient Pharmacists: Dispense medications and pharmaceutical supplies to fill written and e-prescriptions, provide patient counseling, and monitor medication therapy.

Work Environment

• Potential exposure to hazards and risks of the hospital environment, including infectious diseases and hazardous substances.
• Requires the ability to stand approximately 60% of the time and occasional reaching.

Qualifications

• Doctor of Pharmacy or Bachelor of Science in Pharmacy degree.
• Current New Jersey State license as a Registered Pharmacist.
• ACLS and CPR preferred for Hospital Pharmacist roles.
• Two years of experience preferred; two years of hospital experience preferred for Hospital Pharmacist roles.
• Proficiency in Clinical Applications preferred at hire; training and assessment provided within 90 days.
