Outpatient Occupational Therapist (Full Time - 30 hours)

North Country Healthcare (NHC) is a non-profit affiliation of four medical facilities located in the White Mountains Region of New Hampshire, delivering integrated patient care through community hospitals, specialty clinics, and home health and hospice services.

About the role

The Occupational Therapist evaluates and treats various orthopedic, neurological, general, and complex medical conditions in inpatient and/or outpatient settings. Treatment is based on functional outcomes and may include Activities of Daily Living skills, therapeutic activities and exercise, manual techniques, and therapeutic modalities.

Responsibilities

• Evaluate patients with various functional disabilities to achieve optimal functional performance in inpatient, outpatient, nursing home, or home care settings.

• Take appropriate action on all physician orders applicable to specific assignment.
• Administer therapy in accordance with prescribing physician’s orders, New Hampshire Occupational Therapy Practice Act, hospital/departmental policies, and regulatory agency standards.
• Complete all evaluations, re-evaluations, treatment plans, and documentation on a timely basis.
• Assess patients for risk or potential of neglect or abuse; make referrals as appropriate.
• Provide effective pain assessment, prevention, and management; report complaints of pain to appropriate medical personnel.
• Manage individual patient/client schedules.
• Contribute to discharge planning, including home programs, community resources, and referrals for continued treatment.
• Participate in educational in-service activities within the department, hospital, and community.
• Take an active role in student affiliation training programs.
• Provide group or individual activities for swing and ICF patients in response to their physical, intellectual, social, spiritual, and emotional needs.
• Complete activity assessments and weekly summaries as needed; document on the activity participation log.
• Achieve an acceptable level of productivity based on clinical areas worked.
• Provide competent coverage in other Occupational Therapy areas when required.
• Demonstrate technical aptitude, familiarity with computer applications, and willingness to learn software applications.

Requirements

• Bachelor’s Degree in Occupational Therapy from an accredited institution with a curriculum approved by the American Occupational Therapy Association.
• Eligibility for licensure to practice occupational therapy in the State of New Hampshire.
