Responsibilities

• Provide preventive services allowing individuals to stay in their homes and out of nursing home care
• Assess patient needs, plan for, evaluate, and modify care to meet goals of Occupational Therapy interventions
• Collaborate with all disciplines to plan and evaluate team goals for each patient
• Instruct patients, families, and caregivers on the patient's total occupational therapy program
• Advise, consult, and instruct members of the multi-disciplinary team in specific occupational therapy modalities
• Observe and report patient progress and/or changes in patient status to physicians and appropriate multi-disciplinary team members
• Other duties as assigned

Requirements

• Master’s degree from an accredited occupational therapy school
• Good organizational skills
• Good written skills
• Valid NY State OT license
• New graduates encouraged to apply

Pay

All rates offered are based on position, years of applicable experience, and employment type. Rate ranges shown include levels for per diems with no benefits and independent contractors.
