About the role

The Orthopedic Care Navigator facilitates the care of patients and the progress and organization of the Orthopedic Total Joint Program by coordinating patient flow and departmental progress. This role utilizes principles of the nursing process to organize and provide quality, continuous nursing care. The Orthopedic Care Navigator is responsible for developing, managing, and coordinating all activities related to the Orthopedic Program, with a main focus on creating the Joint Center of Excellence.

Responsibilities

• Conduct orthopedic educational programs and monitor their effectiveness.
• Facilitate family involvement by supporting the “coach” role in the joint replacement process.
• Ensure smooth patient flow and make patient care decisions based on urgency of individual needs.
• Coordinate all patient processes from initial pre-op testing and registration to post-op care.
• Serve as a liaison between physician's offices, the hospital, and other facilities for preoperative care coordination.
• Develop, implement, and monitor compliance with multidisciplinary orthopedic care pathways, revising based on patient satisfaction, standards of care, and quality information.
• Anticipate provider needs by preparing patients for surgery.
• Assess, set goals, and prepare, implement, document, and adjust treatment and education plans for each patient based on individual circumstances, medical condition, and age.
• Market the orthopedic program through community outreach, utilizing “Graduates” as ambassadors, and providing evidence-based patient feedback to surgeons and providers.
• Monitor joint replacement clinical outcomes and identify opportunities for improvement.
• Track program growth and fiscal impact on the organization.
• Monitor patient satisfaction through surveys and in-house discharge phone calls to gather feedback on care, outcomes, and the joint replacement experience.
• Organize and lead the Orthopedic Program and Joint Center of Excellence, collaborating with all disciplines regarding patient care.
• Receive information from the multidisciplinary joint care team and drive solutions for identified problems.
• Set an example of excellence in clinical practice and professional behavior, acting as a resource to clinical staff, patients, families, and other customers.
• Mentor employees in the development and enhancement of clinical skills.
• Perform all other duties as assigned.

Requirements

• Completion of a nursing education program approved by a board of nursing.
• Registered Nurse (RN) certification - South Dakota Board of Nursing.
• 1+ years of Case Management Experience.
• 1+ years of Patient Care Experience.

Preferred Qualifications

• Bachelor’s degree in Nursing.

Pay

Starting pay rate range: $32.00 - $40.00 per hour (determined by the knowledge, skills, and experience of the applicant).

Schedule

• Full-time, 40 hours per week.
• Requires the ability to work assigned schedules and hours as determined by the department.

Physical Requirements

Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
