About the role
The role of One Care Plan Advocate involves providing comprehensive support to patients in managing their healthcare needs. This includes coordinating care plans, ensuring timely medication management, and facilitating communication between patients, healthcare providers, and other stakeholders.
Responsibilities
- Develop and implement personalized care plans for patients
- Monitor and adjust care plans based on patient progress and feedback
- Coordinate with healthcare providers to ensure continuity of care
- Manage patient medication schedules and ensure compliance
- Communicate effectively with patients, families, and healthcare teams
- Stay updated with latest healthcare trends and advancements
Requirements
- Bachelor’s degree in Healthcare Administration, Nursing, or related field
- Minimum 2 years of experience in healthcare coordination or patient care
- Strong organizational and communication skills
- Ability to work independently and manage multiple tasks simultaneously
- Proficiency in Microsoft Office Suite
Qualifications
- Current certification in Patient Care Coordination (if applicable)
- Experience with electronic health records (EHRs)
- Knowledge of healthcare regulations and standards
