On-Call Registered Nurse (Hospice)

Hospice RN (On Call) provides compassionate, in-home nursing support for hospice patients during after-hours coverage, with a focus on comfort, safety, and symptom management. Key Responsibilities Triage incoming calls and determine appropriate next steps based on patient needs.Assess changes in patient condition and coordinate timely responses.Make in-home visits as needed to manage pain and other symptoms.Follow individualized hospice care plans and document nursing assessments and interventions.Collaborate with an interdisciplinary hospice team to ensure continuity of care.Educate family members on medications, comfort measures, and end-of-life care. Qualifications & Skills Current RN license with hospice or end-of-life care experience preferred.Strong clinical assessment and critical-thinking skills for rapid triage.Ability to provide empathetic care and communicate clearly with patients and families.Knowledge of symptom management and comfort-focused nursing interventions.Reliable documentation and teamwork with multidisciplinary care teams.
