About the role

Support Coordinators (case managers) serve a caseload of about 35-45 participants with intellectual and developmental disabilities. These participants live in their homes and communities and receive physical, medical, educational, or vocational support to assist them in remaining in their desired settings. The primary role is to plan, organize, implement, and monitor in-home and community-based services. Support Coordinators must be agile and responsive to meet changing participant needs and service requirements, including support during emergent situations and public emergencies such as weather-related evacuations.

For the first 90 days, this hybrid position requires full-time in-office work.

Responsibilities

  • Complete quarterly visits, including required units, observation, monitoring, and monthly contacts.
  • Conduct and complete annual CPOCs (Comprehensive Person-Centered Outcomes and Coordination) according to funder’s timelines.
  • Plan, schedule, and coordinate quarterly meetings with participants and providers.
  • Respond to participant changes in needs by completing and submitting appropriate paperwork.
  • Research, access, and link services for participants.
  • Communicate and collaborate with providers to ensure appropriate service provision.
  • Maintain all responsibilities and requirements regarding critical incidents.
  • Meet with supervisor for weekly supervision, consultation, and participant needs.
  • Attend and participate in weekly staff meetings.
  • Complete required training.
  • Support participants during emergent situations and public emergencies, including weather-related evacuations.
  • Act as a mandated reporter under the Louisiana Children’s Code (Article 610) for incidents of child abuse or neglect.

Requirements

  • A Bachelor’s or Master’s Degree in Social Work from a program accredited by the Council on Social Work Education and two (2) years of paid post-degree experience in providing coordination services; or
  • A Bachelor’s or Master’s Degree in Nursing (RN) currently licensed in Louisiana (one year of paid experience as a licensed RN substitutes for the degree) and two (2) years of paid post-degree experience in providing coordination services; or
  • A Bachelor’s or Master’s Degree in a Human Service-related field (e.g., psychology, education, counseling, social services, sociology, philosophy, family and participant sciences, criminal justice, rehabilitation services, substance abuse treatment, gerontology, vocational rehabilitation) and two (2) years of paid post-degree experience in providing support coordination services; or
  • A Bachelor’s Degree in Liberal Arts or General Studies with a concentration of at least 16 hours in one of the following fields (e.g., psychology, education, counseling, social services, sociology, philosophy, family and participant sciences, criminal justice, rehabilitation services, substance abuse treatment, gerontology, vocational rehabilitation) and two (2) years of paid post-degree experience providing support coordination services.
  • Reliable transportation to conduct participant visits in the field (generally in the participant’s home). Approximately 50% or more of your time will be spent on home visits.

This is a full-time, hybrid position located in Saint Rose Plantation, Louisiana. Easterseals Louisiana is a drug-free workplace; pre-employment and random drug tests will be administered. Candidates must pass a background check and driver’s license check.
