Role Summary

This position requires a registered nurse with a Bachelor of Nursing and a minimum of two years of relevant home care experience. The nurse must hold a valid license and current registration to practice in NYS. Responsibilities include providing comprehensive patient care within a safe, well-maintained medical environment, adhering to safety protocols, and ensuring patient privacy and confidentiality.

Key Responsibilities

• Deliver quality home care services aligned with patient needs and clinical standards
• Maintain accurate and timely documentation of patient interactions and care plans
• Coordinate with healthcare teams to optimize patient outcomes
• Ensure compliance with safety and privacy policies, including HIPAA regulations

Qualifications & Skills

• Valid NYS registered nurse license and current registration
• At least two years of relevant home care experience
• Strong clinical and communication skills
• Ability to work independently and collaboratively in a healthcare setting
• Knowledge of safety protocols and patient confidentiality standards
