About the role

Provide safe, holistic care to patients with chronic illnesses and co-morbidities in a home setting. Responsibilities include patient education on self-management, supporting families and communities, and collaborating with healthcare teams to deliver quality, compassionate, patient-centered care.

Responsibilities

• Deliver patient education on self-management of chronic conditions
• Support families and communities in care coordination

Requirements

• Minimum of 1 year med-surg experience
• Home care experience preferred
• BSN preferred
• Proficiency in technology, IV, and venipuncture skills
• Ability to travel within the service area
• Active RN licensure, valid driver’s license, and CPR certification
