Role Overview
The Registered Nurse provides and coordinates home-based nursing care in compliance with laws, regulations, and policies. They collaborate with healthcare professionals, patients, families, and community resources to ensure continuous, personalized care.
Key Responsibilities
- Deliver high-quality clinical nursing services within scope of practice and infection control standards
- Coordinate care throughout the patient's treatment, from admission to discharge
- Perform clinical assessments per federal/state requirements and payer needs
- Ensure patient eligibility and medical necessity for services
- Develop and modify individualized care plans in collaboration with community providers
- Adjust care frequency and duration as needed with physician approval
- Adapt effectively to diverse home environments regardless of client demographics
Qualifications & Skills
- Current, unrestricted RN license in the practice state
- Valid driver's license, vehicle insurance, and access to reliable transportation
- Preferred: CPR certification, 1+ years RN experience, strong communication, organization, and independent working skills
Pay
$34.05 to $51.10 per hour, based on experience and location.
