Rn home care

Wage rate is determined by education (BSN) and number of years as a licensed RN. Additional 3% added to base rate for BSN or MSN. Additional pay for Hospice certification.

About the role

Provide individualized nursing care and symptom management to hospice patients in their homes while collaborating with an interdisciplinary team. Conduct regular patient assessments, update care plans, and provide education and support to patients and their families. This position may be Full-Time or Part-Time and will be designated at time of employment.

Responsibilities

• Provide individualized nursing care in accordance with Hospice philosophies and standards.
• Conduct nursing assessments, provide education to patients and families, and effectively manage symptoms.
• Collaborate with the interdisciplinary team to coordinate patient care.
• Take call on a rotating basis after business hours and on holidays for patient care issues, making visits as needed.
• Assess patient and family needs (physical, social, emotional, spiritual) with prompt follow-up and referrals.
• Develop and update a comprehensive nursing and team care plan in collaboration with the Interdisciplinary Team.
• Ensure optimal symptom and pain control through attentive examination and follow-up.
• Keep the attending physician informed of patient status and request appropriate changes in care.
• Coordinate with the Volunteer Coordinator to assign, direct, and support volunteers.
• Provide spiritual support through patient's personal clergy or hospice chaplain.
• Educate patients and families on hospice protocol, emergencies, and on-call procedures.
• Solicit help from other team members or agencies as appropriate.
• Help patients and families prepare for death while maintaining hope and encouraging pre-planning.
• Encourage family and friends to participate in patient care, providing education and support per the plan of care.
• Accurately and timely record all phases of patient care, including re-certifications and physician orders.
• Attend funerals when possible, then withdraw from active family support, leaving bereavement to the team.
• Participate in Interdisciplinary Team meetings as a positive, participative member.
• Conduct home supervisory visits on LPNs and CNAs assigned to your patients per policy.
• Accurately count medications and order refills to ensure patients have appropriate supplies.
• Perform death pronouncements following policy, including medication destruction and family support.
• Maintain a patient caseload and provide services per the plan of care.
• Acquire 15 continuing education hours per year (12 must be hospice-specific).
• Supervise, direct, and document patient care activities of CNAs in patient homes monthly.
• Temporarily serve as Team Manager when requested by supervisor.
• Act on physician orders and use nursing judgment to assess critical changes.
• Reach out to management for guidance on undefined practices per state regulations.
• Ensure job responsibilities are met, including attendance and prompt arrival.
• Assist with special projects as requested.

Requirements

• Associate's degree or equivalent from a two-year college or technical school (BSN or MSN preferred).
• One year of related experience.
• Valid Alabama and Georgia Nursing License or Compact license.
• Influenza vaccine during influenza season (November–March).
• Valid driver's license, satisfactory MVR, and proof of insurance per policy.
• Dependable personal transportation.
• Current BLS CPR certification.
• Ability to define problems, collect data, establish facts, and draw valid conclusions.
• Ability to interpret technical instructions in mathematical or diagram form.
• Strong language skills: read, analyze, and interpret professional journals, write reports, and present information effectively.

Physical Demands

• Regularly required to talk or hear.
• Frequently required to sit, use hands to finger/handle, and reach with hands and arms.
• Occasionally required to stand, walk, stoop, kneel, crouch, or crawl.
• Must occasionally lift and/or move more than 100 pounds.
• Specific vision abilities: close, distance, peripheral, depth perception, and ability to adjust focus (including day and night driving vision).
• Potential exposure to hostile environments, cigarette smoke, and pets.

Work Environment

• Frequently exposed to outside weather conditions.
• Occasionally exposed to moving mechanical parts, fumes, airborne particles, toxic or caustic chemicals, risk of electrical shock, and risk of radiation.
• Moderate noise level in the work environment.
• Potential exposure to hostile environments, cigarette smoke, and pets.
