About the role

Assess and evaluate patient needs related to functional status, daily activities, fine motor skills, home assessments, and adaptive equipment. Deliver patient-centered care and work collaboratively within a multidisciplinary team.

Responsibilities

• Provide services within legal scope, following the plan of care, using evidence-based techniques, and collaborating with the healthcare team.
• Evaluate patients’ functional status and coordinate with physicians to develop therapy plans.
• Monitor and report patient responses and condition changes to supervisors and physicians.
• Conduct assessments using diagnostic and prognostic tests to determine functional ability.

Requirements

• Current state licensure in Occupational Therapy
• CPR certification
• Valid driver’s license, vehicle insurance, or access to transportation
• Ability to work in diverse home environments regardless of client background

Preferred Skills

• Ability to manage multiple tasks independently
• Strong communication, organizational, and writing skills
