Role Overview

The Occupational Therapist in Home Health assesses and evaluates patients' functional status, activities of daily living, fine motor skills, home environments, and adaptive equipment needs. They develop and implement treatment plans within legal scope, collaborating with healthcare team members and consulting with physicians. The role involves observing, documenting, and reporting patient responses and condition changes, conducting functional assessments using diagnostic tests, and ensuring evidence-based care.

Responsibilities

• Provide occupational therapy services according to state laws and plan of care
• Evaluate patient needs and develop treatment strategies
• Monitor and report patient progress and responses to treatment
• Coordinate with physicians to develop and adjust therapy plans

Qualifications & Skills

• Current state licensure and CPR certification
• Valid driver's license, reliable transportation, or access to public transit
• Ability to work in diverse home environments and with clients of all backgrounds
• Preferred: multitasking, independence, strong communication, and organization skills
