About the role

Provide home health services, assessing and evaluating patients' functional status, activities of daily living, fine motor skills, and adaptive equipment needs. Develop and implement treatment plans in collaboration with physicians, monitor patient responses to therapy, and ensure all practices comply with state regulations and evidence-based guidelines.

Responsibilities

• Conduct patient assessments in home settings
• Develop individualized treatment plans
• Document patient progress and adjust care as needed
• Communicate with healthcare team members to coordinate quality care
• Perform home safety and accessibility assessments

Requirements

• Current Occupational Therapy license in the practice state
• CPR certification
• Valid driver’s license and reliable transportation or access to public transit
• Ability to work effectively in diverse home environments
