About the Role

As an Occupational Therapist in Home Health, you will assess and evaluate patients' functional status, daily activities, fine motor skills, and adaptive equipment needs, developing personalized treatment plans in collaboration with healthcare team members.

Responsibilities

• Provide therapy within legal scope, following evidence-based practices and care plans
• Evaluate patients' functional and systemic status, documenting for CMS compliance
• Monitor and report patient responses and condition changes to supervisors and physicians
• Conduct assessments to determine functional abilities using diagnostic tests

Requirements

• Current state licensure as an Occupational Therapist
• CPR certification
• Valid driver's license with reliable transportation
• Ability to work effectively in diverse home environments regardless of client background
