About the Role

The Occupational Therapist in Home Health assesses and evaluates patients' functional status, activities of daily living, fine motor skills, and home safety to develop personalized treatment plans aligned with medical conditions.

Responsibilities

• Provide therapy within legal practice scope, following evidence-based techniques and collaborating with healthcare team members
• Evaluate patient needs, document CMS-required data, and consult with physicians on care plans
• Monitor and report patient responses to treatment and condition changes
• Conduct functional assessments using diagnostic and prognostic tests

Requirements

• Current state licensure in Occupational Therapy
• CPR certification
• Valid driver's license, reliable transportation or access to public transit
• Ability to work effectively in diverse home environments
