About the role

The Occupational Therapist in Home Health evaluates and treats patients' functional status, activities of daily living, and adaptive needs, within legal scope and in coordination with healthcare team members.

Responsibilities

• Assess patient needs through evaluations and observations, documenting responses and changes.
• Develop and implement therapy plans in collaboration with physicians, using evidence-based techniques.
• Perform home assessments and recommend adaptive equipment as needed.
• Ensure compliance with state laws and CMS documentation requirements.
• Communicate patient progress and updates to relevant healthcare providers.

Requirements

• Current Occupational Therapy licensure in practice state.
• CPR certification, valid driver’s license, and access to reliable transportation.
• Ability to work effectively in diverse home environments.

Pay
