Role Overview: As a Home Health Occupational Therapist, you will provide personalized rehabilitation services to patients in their homes, helping them regain strength, mobility, and independence. Key Responsibilities Assess and develop therapeutic programs based on patient needs and diagnostic tests.Collaborate with physicians and healthcare team members to create and update care plans.Administer treatments to improve strength, coordination, and functional abilities, including teaching compensation strategies.Review and adjust retraining programs aligned with patient goals and rehabilitation guidelines.Consult with vocational team members to develop plans suited to patient capabilities.Report patient progress and participate in discharge planning and care coordination.Design community reintegration activities and recommend adaptive equipment to enhance participation.Train staff and family members in occupational therapy techniques and supervise therapy assistants and aides.Document patient care, interventions, and evaluations accurately and thoroughly. Qualifications & Skills Current, unrestricted OT license and CPR certification.Valid driver’s license, auto insurance, and reliable transportation.Preferred: Minimum six months of occupational therapy experience, preferably in home health. Additional Information: This patient-facing role requires TB screening and adherence to driver safety policies. Compensation ranges from $92,200 to $126,900 annually, based on location and experience. Benefits include paid time off, 401(k), and wellness programs.
