About the role

Occupational Therapist providing outpatient rehabilitation services primarily in in-home settings for older adults, including private residences and independent living facilities.

Responsibilities

• Conduct assessments
• Develop and implement personalized treatment plans
• Facilitate rehabilitation to restore, reinforce, and enhance functional performance
• Supervise support personnel as needed
• Ensure documentation compliance
• Collaborate with multidisciplinary teams and caregivers

Qualifications

• Bachelor’s degree in an accredited occupational therapy program
• Initial certification from the National Board for Certification
• Valid state licensure
• Reliable transportation
