Role: Occupational Therapist (OTR)
Location: White Blossom Care Center – San Jose, CA
Compensation: 50- $65.00/hour (DOE)
Schedule: Full-Time, Part-Time, or PRN
Program Type: Stable, In-House Therapy

Key Responsibilities
- Evaluation & Treatment: Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions per physician orders
- Clinical Leadership: Supervise and support Certified Occupational Therapy Assistants (COTAs) in compliance with California State Practice Acts
- Interdisciplinary Collaboration: Participate in care conferences, utilization reviews, and rehab meetings to ensure coordinated, patient-centered care
- Documentation Excellence: Complete timely and accurate evaluations, daily notes, progress notes, and recertifications in accordance with OT Board standards
- DME & Patient Independence: Recommend, order, and train patients on durable medical equipment (DME) to promote safety and functional independence
- Education & Discharge Planning: Provide caregiver training and education to families and nursing staff to support positive discharge outcomes
Qualifications
- Valid California Occupational Therapist (OT) license in good standing
- Passion for delivering high-quality care in post-acute and long-term care settings
