  • Current Occupational Therapist license in the state of Oklahoma.
  • Bachelor's degree.
  • Basic Life Support (BLS) Certification Required (Available Upon Hire).
  • Annual Influenza Vaccination or Declination Required.
  • Master's Degree preferred.
