About the Role
The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to improve or adjust daily activities. They collaborate with the healthcare team, modify care plans as needed, and serve as a clinical resource and role model for staff and students.
Responsibilities
- Review and communicate care plans with physicians, ensuring timely documentation.
- Educate patients and families to overcome barriers and promote recovery.
- Document patient visits, assessments, and care plans per regulatory guidelines.
- Coordinate supplies, discharge plans, and community resources.
- Evaluate patient function, observe treatment reactions, and report changes.
- Supervise and instruct Occupational Therapy Assistants and other team members.
- Complete assessments, home safety evaluations, and develop individualized plans of care.
Requirements
- Licensed Occupational Therapist in Illinois.
- Basic Life Support (BLS) certification.
- Valid driver’s license and auto insurance.
- Ability to work collaboratively within an interdisciplinary team.
- Strong assessment, documentation, and patient education skills.
