About the role

The Occupational Therapist at Hartford HealthCare at Home will evaluate patients' level of function and develop individualized treatment plans to promote health and wellness, prevent injury or disability, and improve daily living, leisure, and productivity.

Responsibilities

  • Independently evaluates patients' level of function and develops individualized treatment goals and plans.
  • Delivers Occupational Therapy services that promote health and wellness, prevent injury or disability, improve and develop self-dependency and maintenance, productivity, leisure, perceptual motor function, rest, mental, cognitive physical, sensory-perceptual, psychosocial developmental, and emotional wellbeing.
  • Delegates patient care responsibilities to Certified Occupational Therapy Assistants.
  • Evaluates care plan on an ongoing basis, revising as necessary in accordance with patient’s care needs.
  • Establishes measurable goals with the patient/family based on appropriate patient care protocols and critical paths in order to meet patient needs.
  • Notifies Physician of any changes in status or progress.
  • Provides patient and caregiver education regarding treatment, precautions, and equipment use.
  • Completes all documentation including but not limited to: evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary.
  • Participates in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care.
  • Helps to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources as related to care transition planning.
  • Maintains a clean, neat work environment for patients and staff including adhering to infection control and safety precautions.
  • Restocks supplies as necessary.

Qualifications

  • Bachelor's Degree in Occupational Therapy required.
  • Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy preferred.
  • Previous experience as an Occupational Therapist preferred.
  • Current State of CT Occupational Therapy license required.
