Job Name: Home Health Occupational Therapist
Contract type: Local Contract
Location: Epping, NH (Coverage Areas: Epping Care territory)
Duration: 13-week Contract (Possibilities of extension)
Pay rate: $46-53/hr. On W2 (Pay Rate is based on experience)
Schedule: Mon - Fri (5 days/wk.) 8 AM to 4:30 PM (32-40 hrs. weekly guaranteed)
Mileage: Reimbursement provided (from first patient to last patient)
On Call/ Weekend: No

Job Responsibilities

  • Assesses and evaluates patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.
  • Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
  • Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs.
  • Consults with the physician in the development of the therapy plan of care.
  • Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
  • Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Qualifications

  • Current Occupational Therapy license in the state of practice.
  • Home Health & Homecare Homebase (HCHB) experience highly preferred.
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation.
