About the role
Occupational Therapist in Home Health providing assessment, evaluation, and treatment of patients' functional and daily living needs.
Responsibilities
- Deliver occupational therapy services within legal scope, following the plan of care and using evidence-based techniques.
- Evaluate patient functional status, coordinate with physicians on therapy plans, and document CMS-required data.
- Monitor, record, and communicate patient responses and condition changes to the healthcare team.
- Perform assessments to determine functional abilities, applying diagnostic and prognostic tests.
- Provide care in diverse home settings, regardless of patient demographics.
Requirements
- Current state licensure in Occupational Therapy.
- Valid driver's license, vehicle insurance, or access to transportation.
