Occupational Therapist (Home Health)

About the Role

The Occupational Therapist in Home Health assesses and evaluates patients' functional status, daily living activities, fine motor skills, and home environment to develop and implement personalized treatment plans. They provide evidence-based therapy within the scope of practice, coordinate with healthcare team members, and monitor patient responses to treatment.

Responsibilities

• Perform patient assessments to determine functional needs
• Evaluate physical and cognitive abilities and document findings for CMS compliance
• Develop and adjust therapy plans in collaboration with physicians
• Observe and report patient progress and condition changes
• Provide therapy consistent with regulatory standards and evidence-based practices
• Conduct assessments, document patient progress, and consult with physicians to create effective care plans

Requirements

• Current Occupational Therapy licensure in the specified state
• CPR certification
• Valid driver's license, vehicle insurance, and access to reliable transportation
• Ability to work effectively in diverse home settings
