Occupational Therapist - Home Health

Responsibilities

• Assess and evaluate patients' functional status, daily activities, and occupational therapy needs in accordance with medical conditions and plan of care
• Provide services within scope of practice, using evidence-based techniques and collaborating with the healthcare team
• Monitor patient responses to treatment, document progress, and communicate changes to supervising nurse and physician
• Conduct comprehensive patient assessments, including diagnostic and prognostic functional ability tests

Qualifications & Skills

• Current Occupational Therapy license in the practice state
• CPR certification and valid driver's license with reliable transportation or access to public transit
• Ability to work effectively in diverse home environments regardless of age, race, or socioeconomic status
• Strong communication, organizational, and independent working skills (preferred)
• Capability to manage multiple tasks simultaneously (preferred)
