Occupational Therapist - Home Health
Responsibilities
- Complete Start of Care (SOC) visits
- Perform Recertifications and Discharge visits
- Assess patients and develop individualized care plans
- Provide skilled care and patient/caregiver education
- Maintain accurate clinical documentation
- Collaborate with the interdisciplinary care team
Requirements
- Active OT license
- Home Health experience required
- Strong communication and patient care skills
- Valid driver's license and reliable transportation

Pay
$51 to $54/hr on W2
Schedule
Monday-Friday | 8:00 AM - 4:30 PM
