Duties
- Provide occupational therapy services through in-home patient visits.
- Evaluate and treat patients in their home environment.
- Develop and implement appropriate occupational therapy treatment plans.
- Monitor patient progress and document patient care accurately.
- Provide care using HCHB and maintain a productivity expectation of 30 points.
Requirements
- Education: Graduate of an accredited Occupational Therapy program
- Licenses: Active Occupational Therapist (OT) license
- Certifications: OT license
- Experience: Home Health experience required

Pay Range: $52.5/hr on W2
*Pay offered is based on experience, expertise, credentialing, and education.
