Occupational Therapist - Home Health

Pride Health is hiring an Occupational Therapist for a 13+ week contract (with possibilities of extension) in a home health program serving patients in Portland, Oregon.

Responsibilities

• Perform occupational therapy evaluations in patients’ homes
• Develop individualized treatment plans based on patient needs and goals
• Implement therapeutic interventions to improve mobility, strength, coordination, and daily living skills
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapy techniques, safety practices, and adaptive equipment
• Collaborate with physicians, nurses, and interdisciplinary care teams to ensure quality patient care
• Maintain accurate and timely documentation in accordance with agency and regulatory standards

Requirements

• Current Occupational Therapist licensure
• Home health/Home care experience
• Current CPR/BLS certification
• Valid driver’s license
• Reliable transportation

Pay

• Local Pay: $55.00 - $57.00/hour (W-2)
• Travel Pay Package: Up to $2,536/week gross (includes weekly non-taxable stipend)
• Mileage reimbursement provided (from first patient to last patient, excluding travel to and from home)

Schedule

• Monday-Friday, 8:00 AM-4:30 PM
• No on-call or weekend shifts
