Occupational Therapist - Home Health

About the Role

This position involves providing personalized occupational therapy services in patients' homes to promote recovery, independence, and improved quality of life.

Responsibilities

• Assess and screen patient skills, develop measurable therapeutic programs
• Coordinate with physicians and healthcare team to develop and revise treatment plans
• Provide therapeutic treatments to restore or improve function
• Review and adapt retraining programs aligned with patient goals
• Consult with vocational team members for occupational plans
• Report patient progress and participate in discharge planning
• Design community reintegration activities and recommend adaptive equipment
• Train staff and family members in therapy techniques
• Supervise and evaluate therapy assistants and aides
• Accurately document patient care and interventions

This role is patient-facing, requiring adherence to TB screening and driver safety programs.

Requirements

• Current and unrestricted OT licensure
• CPR certification
• Valid driver’s license, auto insurance, and reliable transportation

Pay

Compensation ranges from $76,900 to $105,800 annually.
