About the role
We are seeking a dedicated Home Health Occupational Therapist to provide personalized therapy services in patients' homes.
Responsibilities
- Assessing patients' daily living and work skills
- Developing and implementing treatment plans
- Collaborating with physicians and healthcare teams to optimize patient outcomes
- Delivering therapeutic interventions to improve strength, coordination, and mobility
- Educating patients, families, and staff on rehabilitation techniques
- Supervising therapy assistants
- Documenting care activities accurately
- Designing community reintegration activities
- Recommending adaptive equipment
- Participating in discharge planning and care coordination
Qualifications
- Valid OT license
- CPR certification
- Reliable transportation
Preferred experience
- At least six months in occupational therapy
- At least six months in home health
