Occupational Therapist (Home Health)

Role Overview

Responsible for planning and conducting treatment programs aimed at restoring and enhancing patients' physical, social, and mental functions. Acts as a co-leader within an interdisciplinary team to deliver personalized in-home care.

Key Responsibilities

• Perform comprehensive evaluations to develop individualized treatment plans using appropriate clinical techniques and tools.
• Provide therapy interventions following established protocols, ensuring clear communication of care and discharge plans.
• Complete accurate clinical documentation within designated deadlines.
• Supervise assigned therapy assistants and collaborate with team members.
• Perform additional duties related to patient care as needed.

Qualifications & Skills

• Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD), with licensure in applicable states.
• Valid occupational therapy license, CPR/BLS certification, and a valid driver's license.
• Strong clinical assessment and documentation skills.
• Ability to work collaboratively within an interdisciplinary team.
